Understanding cellulite structure

Cellulite is a structural problem — not simply excess fat. The female pattern of septae runs perpendicular to the skin surface (vertical), while men have a criss-cross pattern that keeps fat from pushing through. When fat expands or the septae become tighter and more fibrotic with age, the fat bulges between bands, pulling the skin surface downward into dimples.

This explains why cellulite cannot be treated with diet alone — the problem is the structural interplay between fat and fibrous tissue, not fat quantity per se.

Cellulite is graded 1-3 (Nurnberger-Muller scale). Grade 1 (visible only with skin pinch) responds best and most consistently to treatment. Grade 2 (visible standing, not lying) shows good improvement. Grade 3 (visible lying down, severe dimpling) shows partial improvement — treatment reduces severity but may not eliminate higher-grade cellulite.

Cellulite tends to recur because its structural cause (fibrous septae and fat) is not permanently resolved. Ongoing maintenance — periodic RF or AWE sessions every 6-12 months combined with consistent exercise, hydration, and retinoid use — sustains the improvement achieved during the initial treatment series.

Not necessarily. Weight loss can reduce fat volume and lessen the severity of cellulite, but the underlying fibrous septae structure remains. Thin patients can still have prominent cellulite — structural treatment is needed regardless of weight.

OTC creams with caffeine, retinol, or aminophylline produce mild, temporary improvements in circulation and skin firmness but cannot address the underlying septae structure. Medical-grade retinoids used consistently provide the most genuine topical benefit.

Most patients describe a warm, deep heating sensation that is generally comfortable. Some mechanical RF devices combine massage, which can produce pressure sensations in sensitive areas such as the inner thigh.

A typical series is 6-8 sessions for meaningful improvement, often biweekly. Maintenance sessions every 2-3 months sustain results. The more consistent the treatment, the better the long-term outcome.

Subcision is a minimally invasive procedure using a small needle or blade under local anaesthesia to release fibrotic bands causing deep dimples. After numbing, the procedure is well-tolerated. Results are among the most lasting for localised, deep dimples.

Yes. While cellulite affects women predominantly, men with cellulite respond equally well to the same treatments. The underlying mechanism and treatments are the same regardless of gender.

RF and AWE are very low-risk — temporary redness and mild soreness are the most common side effects. Subcision involves a small risk of bruising and uneven result if performed without adequate technique — experience of the provider matters greatly.

Exercise reduces overall fat mass and builds muscle that provides structural support, which can reduce the appearance of cellulite. However, exercise alone cannot dissolve fibrotic septae. Combined with clinical treatment, exercise significantly amplifies and maintains results.
